I agree to comply with all COVID-19-related procedures, policies, rules, instructions and signage while attending the event.

I represent and attest that I will only attend the event in-person if all of the following are true:

  1. I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
  2. I don't believe I have been exposed within two weeks prior to the event to someone with a suspected and/or confirmed case of the COVID-19.
  3. I haven't been diagnosed within two weeks prior to the event with COVID-19.
  4. I have been fully vaccinated.

I hereby release, waive, discharge, and covenant not to sue Sparsha Arts Foundation, its officers, directors, employees, volunteers, independent contractors (collectively "Releases") from any and all liability, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage or injury, including death, that may be sustained by me related to COVID-19 whether caused by the negligence of the Releases or otherwise while attending the event.
